Engagement Agreement
Client Responsibility
I freely consent to work with Kelly Dignan (also known as "AWARE CO, LLC”). I am responsible for my own growth. Services provided by Kelly Dignan are not psychotherapy or counseling. They do not address mental disorders as defined by the American Psychiatric Association. My sessions with Kelly are not a substitute for counseling, psychotherapy, mental health care, substance abuse treatment or medical care. If I am struggling from a medical or psychological perspective, I will access professionals in these fields. I am 100% responsible for myself, my choices and behaviors.
Confidentiality and Reporting
Information provided during sessions is confidential unless there is reasonable cause to know or suspect self-harm, that a child or elderly person has been subjected to abuse or neglect, or that a child or elderly person is being subjected to circumstances or conditions that would reasonably result in abuse or neglect. This is a professional relationship in which sexual intimacy is never appropriate.
Insurance, Payment
I understand there is no insurance coverage for these sessions. Payment will be made to Kelly Dignan (aka AWARE CO, LLC) within 24 hours. There are no refunds for sessions held. Until all past sessions are paid for, no further sessions will be held.
Cancellation
If I need to cancel or reschedule a session, I will email Kelly with at least 24-hour notice at [email protected]. Cancellations without 24-hour notice, or no-shows, will be charged in full. Sessions that start late will be charged in full.
If (and only if) I have agreed to a "pack" of sessions, I can pay as a I go, and I have three sessions to decide if Kelly and I are a fit. After that, if I wish to cancel, a $100 cancellation fee will be charged. All packages must be used within a calendar year.
Acknowledgment and Release of Liability
I acknowledge that I understand the above information. Furthermore, I agree to hold harmless Kelly Dignan and AWARE CO, LLC from all liabilities and claims which may arise as a result of services rendered.
